The achievement is to "shake a spot." This means that when you are spotted, you go into cover for a time to lose the spot. Your spotted status is to the right of your compass. A closed eye means that you are not seen, a half open eye means that you are in the line of sight of an enemy, but you are not spotted, and an open eye means that you're spotted.
